They have a subtle difference and they have a variety of uses... if you could flesh out your question a bit more, I can adjust my answer to better suit your needs.

Identify is used when you're trying to figure out what something is:

How do you identify if a shape is a square?

Identify this state.

Distinguish has more of the implication that you're trying to separate two (or more) things:

How do you distinguish between a square and a hexagon?

Can you distinguish green from red? If not, you may be colorblind.
